To use Patient Access you will need a Practice ID number and Access ID number which
is obtained from the practice, and, in conjunction with a password, will be unique to you.

How to register to use the Internet Appointment Booking and Repeat Prescription Facility
To use the Internet Appointment Booking and Repeat Prescription facility you need to register on the web site at http://www.patient.co.uk/.
There are two registration screens. The information you enter on the first screen enables the system to check your identity when you log on. On the second screen you enter a password and security details that no-one else will know.
Look after this form carefully until you register, and consider destroying it afterwards. If you lose the form or if it is stolen before you have entered your password and security details, contact the practice straight away, by telephone or in person.
Remember to enter the information exactly as it appears on this form, or your registration will not be accepted. When you log on after registering you must also enter all details in exactly the same format.
Once you have accessed the screen you will be able to book an appointment with the Doctor, inform us of Changes of Address/Contact Number and order your repeat prescription. Please only book routine appointments. If you need an appointment for a smear, minor operation or any medical assessment, such as a driving medical, please contact the surgery in the usual way.
As of April 2016 you can access the following online:
- Make appointments
- Order Repeat Prescriptions
- View your immunisations and allergies
- See your test results
- Access hospital letters
To access the above please complete the Patient Online Access Registration form (see link below) or alternatively the form can be emailed or you can collect one next time you visit the Surgery. Once we receive your completed form you will be issued with a password and PIN number this will enable you to access appointments, repeat prescriptions and immunisations/allergies immediately. It takes approximately 2 weeks for your test results and hospital letters to upload (this depends on the size of your records and is usually uploaded within a couple of days but may take up to 2 weeks).
